About Bridge Care & Support
Bridge Care & Support is a nurse-led care and support service. We use the nursing experience of our directors to address the increasingly complex needs of individuals being cared for in the community, who often have multiple chronic conditions.
Services are designed to maximise independence, building on the strengths of each individual to rebuild skills, live safely in their own home and maintain social and community connections.
We pride ourselves in our flexible and co-operative approach to our work to ensure the people who use our services receive a high quality service. Our services are registered with the Care Quality Commission ( CQC).
